The CHD Transition Workshop is a supportive online session for young people with congenital heart disease (CHD) who are approaching the move from paediatric to adult healthcare.

This workshop is designed to help you feel more prepared and confident about what transition means. We’ll talk through what to expect, explore ways to build confidence, and support you to develop self-advocacy skills — helping you feel more comfortable speaking up, asking questions, and taking an active role in your care.
