Transition isn’t just about turning 18 — it’s about learning more about your heart, building confidence, and preparing to take the lead in your own care.

We’d like to invite you to our online CHD Transition Myth Buster, an interactive workshop designed for young people aged 12-17.

We’re stripping away the rumors and giving you the straight facts about what transitioning to adult care actually looks like, tackling common myths and misconceptions and reducing any worry or uncertainty.
